Juniper’s solution is care that is both high tech and high touch. We have developed Connect4Life to integrate and coordinate individualized care and service. Connect4Life has four key components:

  • Intentional use of data and outcomes to drive quality, define performance and guide operations. Each Juniper community documents and trends more than 90 measures across five domains, many focused on physical health intensities and conditions.
  • Integration of primary care clinics directly in senior living communities. These services are based on the patient centered medical home model and provided via Redwood Health Partners.
  • Cultivating well-being through therapy driven physical wellness programming (Theralink, a joint venture with Genesis Rehab), as well as through seamless access to other services via strategic partnerships and alliances.
  • Streamlining and increasing quality measures with an emphasis on medication administration through integrated bi-directional pharmacy.
